AFP at any age is a prognostic marker and not a diagnostic test. A progressive rise in AFP from previous high level in a neonate is also a hint of malignancy and should be imaged appropriately to look for the source of malignancy. AFP is high not only in germ cell tumours but hepatoblastoma too.
