This patient has a direct inguinal hernia. This is characterized by a hernia defect medial to the epigastric vessels.

Short arrow - epigastric vessels

Long arrow - testicular vessels and vas deferens entering internal ring

In an infant the best option for the management of this hernia repair is a tissue repair to close the hernia defect. A high ligation would be insufficient to repair the direct hernia and on laparoscopic evaluation there is no evidence of an indirect hernia (defect lateral to the epigastric vessels). A mesh repair would be contra-indicated in a patient of this age.
