    quiz trauma with splenic injury

First one with correct answer with justification win.

A two-year old girl is injured in a motor vehicle crash. A computerized tomography scan demonstrates a grade V injury of the spleen and free air. At laparotomy, an isolated jejunal injury is repaired primarily. The spleen is bleeding from several cortical lacerations but the hilum is intact.

The next best step in management of this patient with a splenic injury is

    A splenectomy.

B suture ligation of the splenic capsular tears.

    C packing and re-exploration in 48 hours.

D polyglycolic mesh wrapping of the spleen.

    E segmental splenectomy.

          Splenectomy is the correct choice.
splenectomy is indicated for splenic injury grades 3 to 5

in this scenario, the injury was grade 5 on CT, but on exploration hilum was intact. in children every possible effort should be done to preserve spleen.
