In my training, I learned to always perform derivations, it wasn't a question. However, since I became a surgeon, I started observing a surgeon who almost always performed anastomoses and it worked for him. Currently, I perform the anastomosis beforehand and I am very close to the patient's evolution; it's definitely yielding better results.
