interesting_case Testicular feminization syndrome

I came across a 4 years old girl with bilateral inguinal hernia. On laparoscopic inspection
turned up to be testicular feminizations syndrome. The familiy are simple illiterate farmers
The question is
do you tell the family that their beautiful girl is a male?!
will the simple family understand and be able to cope.
or there is another Way to communicate the information.

Dear sir, male conversion will be really difficult in this child and because personally, socially has been reared as a girl, removal of the gonads and if posible a vaginal reconstruction should serve all practical purposes I feel. All the best.

Yes I agree to keep her as a girl. But The conversation will be really difficult. one of my prof said that he had one case before of 4 children with in the same family and family wanted them to be males but the children didn’t accept it. End up causing them suiside.
I think this decision is really hard and talking to a psychologist will help the family and the kid.

Go for karyotyping genetics and endocrine consultation...after concluding the case u have to discuss it with family along with psychologists to overcome the trauma

The point I was raising hiw much information should be commicated with uneducated families. Do we go all the way to explain the genitic backround of being a male where it is uptill now impossible to make a decent functioning penis ;or simply to tell them that they've a daughter that will not be able to have children. ?!

It is really difficult conversation and I think it will help to get opinion from a psychologist he may help on how to break a huge news.
when I was in Pakistan we didn’t had this option easily and it helped to have a male member of a family . Away from the child , we break it with the fact which we found parents will get a confused idea but it ease the way through . It may also help to give the news in different stages , we tell our expectations and confirm with fact in 2 to 3 session.

The point I want to raise is that up till now there is no way that testicular feminization patients can be reered as male as there no possibility to have a functional penis. at least not to my knowledge. and almost all of them are brought up infertile females..
in rural and tribal communities it is virtually impossible to explain the genetic background of those patients. I have been in such a difficult situation twice in my practicing career and the reaction of the family to my attempt to explain to them that their beautiful girl is in fact a boy!!!!????
What i need to know has anybody passed through a similar experience that he wants to share with me?
