    interesting_case Delayed presentation of Adrenogenital syndrome

    I have a 15 years old boy from Afghanistan who is the only Male in a family of 7 girls. He also was engaged to get married next summer He was referred because of sudden hematuria which on investigation proved to be the menstrual blood and finally diagnosed as adrenogenital syndrome with complete virilization. He has very good size penis . The question I would like to ask what is best for the child
    to be reared as the real female sex or to rear him as a boy (functional and social sex)???

    Very interesting case though!!! On one hand, the person's self esteem is male, psychologically and socially too. Although, internally all functioning female organs are expected. The desires of the person's self is very vital. I would find more rational, removal of internal organs, preservation of phallus, and testicular implant after consulting with the index person and family.

      After long discussion with the family and the boy. There was no way that we can convert him to female sex. So we opted to remove the female structure. We had a promise from the father to be honest in conveying the message to the bride family that ther is no chance if having children fir the neer future at least.
